BEFORE SCHEDULING AN APPOINTMENT
You will need a physician’s order from your doctor. If you do not have a physician’s order, contact your doctor’s office. Appointments cannot be scheduled without an order from your doctor. Your doctor’s office typically will not send an order the same day you have seen him or her.
Your doctor’s office can fax orders to scheduling at (225) 765-9808.
For tests and procedures which require pre-certification, ensure that your doctor’s office staff has contacted your insurance company. Appointments cannot be scheduled without required pre-authorization from your insurance company. Have your insurance card and your social security card with you.
